Rachel Pellum, APRN - President and CEO

Rachel has been providing in home primary care to homebound patients since 2013 as a nurse practitioner. Prior to making medical house calls, Rachel cared for college students at the student health center and taught for the school of nursing at UNF. As a nurse, she specialized in oncology and med-surg nursing. She is a graduate of UNF receiving BSN in 2005 and MSN in 2010. As well, she is a graduate of LSU with a BS in secondary education in 2000.

Nathan Pellum, RN - Vice President 

Nate has been a home healthcare nurse since 2014, visiting and caring for homebound patients. Prior to home health nursing, Nate cared for patients on a post-surgical orthopedic/gastroenterology floor for 6 years. He has an extensive past career in the pre-press/print industry working for Florida Color Graphics for 24 years progressing from an equipment operator to Vice President of the company.

Mary Ann Cox, BCBC - Operations Manager and CFO

Mary Ann has been providing oversight and management in the free medical clinic sector since 2008. She has served as Executive Director and has been instrumental in starting as well as growing these vital services in two communities. Most recently she worked in the house call industry, developing and overseeing provider support programs. She has extensive experience launching and managing several non-profit and church programs.

Brooke Brown, APRN - Primary Care Provider

Brooke has been providing primary care since 2016 as a nurse practitioner and began caring for patients as a registered nurse in 2011. She graduated Summa Cum Laude from UF, followed by receiving her BSN and MSN from UNF.  She is an enthusiastic and dedicated Family Nurse Practitioner with diverse experience. She has a background in family practice, gastroenterology, skilled nursing facilities, women's health, and cardiac nursing.

Krista Collins, APRN - Primary Care Provider

Krista has been a dual-certified Nurse Practitioner since 2004. She received her BSN in 1999 from JU followed by her Master of Science-Perinatal Women's Health CNS/NP from Georgia State University. She has achieved tremendous success as a nurse practitioner and has experience in housecalls, palliative care, internal medicine, family practice and public health.

Tiffany Terpstra, APRN - Primary Care Provider

Tiffany brings nine years of nursing experience providing high quality patient care. Prior to joining the Caritas family, Tiffany worked in a skilled nursing facility in Houma, LA. She received her BSN from Indiana Wesleyan University and her MSN from Chamberlain University.  She has had experience in family medicine, hospitals, home health, emergency medicine and skilled facilities.

Katrina Kutta, APRN - Primary Care Provider

Katrina returned to Florida from St. Louis Missouri where she spent five years at Barnes Jewish Hospital. She received her BSN from Duquesne University and her MSN from University of Missouri Kansas City. She joined our Caritas family, excited to have the ability to positively impact her patients health and quality of life.

Erwin Torralba, APRN - Primary Care Provider

Erwin is originally from Northeast Florida and brings experience working as a nurse practitioner in a local cardiology and vein clinic. He is a graduate of Jacksonville University, receiving his BSN in 2007 and his MSN from Chamberlain University.  Erwin has many years experience working in intensive care, progressive care and cardiac care units.

Erin Crews, APRN - Primary Care Provider

Erin joined Caritas in the Spring of 2024. She completed her BSN at SJRSC and her MSN at University of St Augustine. Prior to joining us, Erin spent eight years providing excellent patient care at Putnam County Medical Center. She is looking forward to caring for those in the Putnam county community and we are thankful to have her on our team.

Amy Decker, APRN - Primary Care Provider

Amy has been caring for patients for more than ten years. Prior to joining Caritas she worked as a Nurse Practitioner in Oncology at BMDA. She received her BSN in Jacksonville University and her MSN from Florida Southern College.  She joined the Caritas family because she is passionate about helping patients achieve the best quality of life in their home.
