In-Home Primary Care Services

• Primary Care visit with Nurse Practitioner

• Diagnosis and Treatment of Acute and Chronic Illnesses

• Medication management

  • Refilling, starting, and discontinuing medications as appropriate​

  • INR management of Coumadin

• Diagnostic and lab testing: Coordination for in-home imaging/testing or referral to outpatient center

• Hospital/Rehab Transitional Care

• Home Healthcare: Coordination and oversight

  • Therapy, Nursing, Social Worker​

• Specialists:

  • Collaboration with Specialists

  • Referral to New Specialists as appropriate 

• Medical Equipment: Ordering through insurance

• Assisted Living and Skilled Nursing/Rehab: Coordinate transition assessment and paperwork

• Hospice Palliative and End of Life Coordination

 

Locations We Serve

We serve home-bound/home limited patients in their homes, assisted living and independent living facilities in Clay, Duval, Putnam, and parts  of Marion county in Florida.

Insurance Accepted

Home visits is a Medicare covered service.  We currently accept most insurance with home-bound benefit, please call to verify for in-network and out of network coverage.

 

Self Pay Services

When in-home primary care is needed, but Patient does not meet home-bound/home limited qualifications necessary for insurance billing a self pay options is available.  Call for more details.
